How do I get them boxes?

The credits of most recent author and previous contributors is added automatically to every page.
The "Work in Progress" box you get by adding [[category:work in progress]] anywhere in your document.
The "To Be Deleted" box is from adding [[category:delete]] anywhere in your document. It will always be displayed after "Work in Progress" box, if there is one.
The "Language Support" box you get by adding {{LanguageSupport|code=en-us, en-pg, zh-tw, ja}} at top of your document. See this document for more information. If there are other information boxes, they will be displayed before this one.

There is no page for testing wiki edit functions, so here it is.

Just installed the program to make this work. Instead of using <math> tags, please use <tex> tags. I've also modified the edit buttons to insert the correct tags.
